Appointments
DCHC asks that you attend all scheduled medical appointments. If you are unable to attend your scheduled appointment, or if you are going to be late, please call the center. Patients arriving more that 20 minutes late for their appointment may be rescheduled.

Please Bring
DCHC asks that all patients bring valid proof of identification. Please provide DCHC with your current insurance information. DCHC will require current verification of your household income in order to determine payment Please come prepared to pay any fees or copays at the time service is rendered.

Emergencies
In the event of an emergency after the center is closed, please call your clinic's direct line. DCHC's answering service will contact your doctor for you. If you are admitted to the hospital, your DCHC family doctor will take care of you during your entire stay from admission to discharge.

Referrals
DCHC will provide referrals to a specialist as needed. Through our partnerships with Detroit health care providers, you have access to over 40 medical specialists. DCHC asks that all patients set up an appointment with their primary care physician to request referrals., Al referral requests should be made in a timely manner, allowing for adequate processing time before your specialty appointment.

Services
In addition to our spectrum of primary medical services, DHC offers you dental care, Family Planning, HIV/AIDS testing, counseling, and treatment, and access to a full-service pharmacy. Please ask any of our friendly staff for information about these important services.
