Appointments:
DCHC asks that patients honor all scheduled
medical appointments. If a patient is unable to
attend a scheduled appointment, or is expected
to be late, they are asked to call the center.
Patients arriving more than 20 minutes late for
a scheduled appointment may be rescheduled.
Required
Information: DCHC asks that all
patients bring valid proof of identification to
their appointments or visit to the center.
Patients are asked to also provide DCHC with
their current insurance information or status.
In order to determine payment, DCHC asks for
verification of the patient’s household income
and that patients come prepared to pay any fees
or co-pays at the time of service.
Referrals:
DCHC will provide referrals to specialists, as
needed. Through partnerships with other
Detroit
health care providers, patients have access to
more than 40 medical specialists in a variety of
areas. DCHC asks all patients to schedule an
appointment with their primary care physician to
request referrals. All referral requests should
be made in a timely manner, allowing for
adequate processing before the patients
appointment with a specialist.
Services:
In addition to a full-spectrum of primary
medical services, DCHC also offers quality
dental care, Family Planning, HIV/AIDS testing,
counseling and treatment, and full-pharmacy
services. Any of the DCHC staff will gladly
provide information about these or other
important DCHC services.
Emergencies:
In the unfortunate event of an emergency after
the center is closed, patients are asked to call
their center’s direct line. DCHC’s 24-hour
answering service will contact the patient’s
physician on their behalf. If a patient is
admitted to a hospital, their DCHC physical will
provide patient care for their entire hospital
stay, from admission to discharge. If an
emergency is life threatening, patients should
call 911.
